In order to serve you better and faster we have provided our new patient forms online. For ease and convenience, new patients can fill out this form in advance of their visit. This will save you time and allow you to fill the form out accurately and completely.

Thank you for choosing Alexander Orthopaedic Associates  as your orthopaedic health care provider.  We welcome new patients to our practice. Being referred to an orthopaedic surgeon suggests that you or your family member probably have been experiencing moderate to severe discomfort.  We try to recognize that this may be an uncomfortable time for you and will make every effort to help. Besides providing the highest quality orthopaedic care possible, we place a great deal of emphasis on education and information regarding your condition. 
what to bring
To better understand your medical history and the treatment you have had to date, it is vital that you bring several items to your first appointment. 
  • New Patient Form (if you have downloaded it from our site)
  • Medical records
    Knowing your medical history and what treatments you have had to date will help us tremendously with his initial evaluation. When possible, have your doctor make copies of your medical records (hospitalizations, office visits and treatments) and bring them with you.
  • Referring physician information
    Dr. Alexander will want to consult with your primary care physician regarding your disease to share the findings so that you may be offered the best course of treatment. Please bring the addresses and phone / fax numbers of your primary care physician with you to your first appointment. If you would like a copy of your report forwarded to other physicians, let us know their contact information.
  • Medications
    Any medications that you are taking should be brought with you to your first appointment, or at least a listing with dosages.
  • X-rays
    Arrange to bring the actual x-ray films related to your medical condition  to your first appointment. You can obtain these from the radiology department where your films were taken. This will save time and avoid having to have the x-rays redone.
  • Insurance pre-authorization form and ID card
    Your health care insurance coverage may have been discussed before you arrive, but please bring your health insurance ID card, along with your referral pre-authorization form (unless we already have the necessary authorization)
  • Workers Comp or auto claim information. (if this is the reason for your visit)
